Activation Fee $150, includes delivery of modem loaned for duration of service.

Modem Usage Fee $50.00 (waived on orders signed and delivered through 08/30/03). Modem provided is a new or fully inspected, tested, warranted return unit. Modem must be returned upon completion of service or Customer will be subject to $200 penalty.

No-Risk savings plan (Optional), (Available on all speeds) 7.5% off for 12 month advance payment; 15% off for 24 month advance payment.

TERMS AND CONDITIONS:

Payment of activation fee, attended installation fee (if applicable) and first period’s monthly recurring charge must be made BEFORE activation. Payment of monthly recurring charge must be made in full on time in accordance with invoices presented by Stickdog to customer.

Service is billable upon activation of modem. Configuration of customer’s computer and network equipment are the responsibility of customer. Stickdog can assist with computer and network configuration upon customer’s request and subject to Stickdog’s normal charges for these services.

Disconnection requires 30 days notice, counted from the first business day after Stickdog receives customer’s request to disconnect. Customer remains liable for charges through the 30 day notice period.

Services not available to all areas. Availability of service and the actual data transfer speed that you experience are subject to various limiting factors, including distance between your location and the central office serving you, the condition of your telephone line and wiring inside your home or office, network or Internet congestion, and server and router speeds for the Web sites you are accessing. Stickdog cannot guarantee uninterrupted, error free or specific-speed service, but offers this service on a best efforts basis.

Subscriber agrees to use this service in compliance with all applicable laws and to indemnify and hold Stickdog harmless for any loss incurred by Stickdog or Stickdog’s suppliers as a result of Subscriber’s use of Stickdog’s service. Subscriber agrees to pay fees in accordance with Stickdog’s rate schedule listed above, and revisions thereto, provided that Stickdog provides notice to Subscriber of any rate increase at least 60 days prior to next payment due date.
